- First mention of Dr. Mickhead
- Dr. Cox threatens Dr. Norris (Christopher Meloni) by kidnapping his puppet and sending the hand in a box. Meloni was in the HBO series Oz, a prision drama which featured a plotline in its 4th season about one character kidnapping another prisoner's children and mailing the son's hand back to the father. Oz ended after 6 seasons in February 2003, during Scrubs' 2nd season.

My Brother, Where Art Thou?

- J.D. calls Mr. Bober, the elderly man who keeps getting shuffled back and forth from his nursing home to the hospital, a "gomer." When J.D.'s brother Dan asks what a "gomer" is, J.D. explains that it's an old person that just keeps taking up a bed because he doesn't have the decency to just die already (showing that J.D. has become jaded and cynical and inspiring Dan to confront Dr. Cox about being a better role model for J.D.). Mr. Bober was the same patient in the Season 1 finale that caused J.D., Elliot and Turk to realize that they had become cynical over the past year due to the horrible way they had treated Mr. Bober (passing him off to one another, playing rock-paper-scissors in the lobby to determine who gets "stuck" with him and almost having his granddaughter overhear them and finally realizing that even though they had all had him as a patient, they didn't even know what he looked like).
- Early in the episode Zach Braff (J.D.) can be seen wearing a red and black Flash t-shirt. Tom Cavanagh who plays his brother in this episode, plays Dr. Wells in The Flash (2014) TV show.
- Second appearance of Dan Dorian.
My Advice to You

- JD says "Are you stalking to me?" to Danni (Tara Reid). Ryan Reynolds, who also plays a character on Scrubs, says the same thing to her in Van Wilder.
- Does not have the usual beginning sequence. Just a shot to title with the sung words of, "I'm no Superman." This is the first episode to do this.

- J.D. thinks that the Janitor is in the movie 'Fugitive, The (1993)' (with Harrison Ford). Real footage from the movie is used in this episode, and in fact actor Neil Flynn (Janitor) plays the role of a cop in the scene.
- The scene where the car drives off the bridge is from Vanilla Sky (2001).
- Dr. Reid's patient, Mr. Moran, is played by Bernie Kopell. Kopell played the character Dr. Adam Bricker (Doc), on The Love Boat.
My Dirty Secret

- Barry Bostwick is the fourth cast member of Spin City, creator Bill Lawrence's previous show, to make a guest appearance.
- In the Netflix streaming version of this episode, the song "Have you ever really loved a woman" was changed to another song.
- Dr. Cox asks a couple if they're WASPs, which stands for White Anglo-Saxon Protestant, and is a somewhat derogatory term.
- This episode was aired out of order, as evidenced by Sean being in town (not on his long-term expedition) and JD being single (not with Danni).

- Michael J. Fox is the fifth member to appear from the TV show Spin City (1996), which had been Bill Lawrence's previous home before creating Scrubs. Also, Martin Klebba plays a pint-sized janitor named Randall Winston, another one of Lawrence's characters from Spin City.
- When Dr. Kevin Casey and Dr. Cox are competing in front of the interns, Dr. Casey, in an attempt to be excused from the game, turns to Dr. Cox and says he need to go home and "make" (go to the bathroom). The gesture he uses as he is saying this is "make" in American Sign Language (though NOT the sign for going to the bathroom).
- The pen that Dr. Kevin Casey (Michael J. Fox) is using when he writes on the charts and simultaneously repeats his name is called a Morph, manufactured by A.T. Cross Co.

- Dr. Cox asked Ben when will he stop carrying around that stupid camera, where he replied when I'm dead. Once Dr. Cox is told about the death and Ben reappeared having no camera.
- There were a few signs leading to Ben's death: Ben only interacts with Perry after the announcement; the unusually passive-aggressive blaming by Perry, even though there were deaths caused by J.D. in prior episodes; the camera missing since the announcement, when Ben said the camera will leave him "when I'm dead".
- Since the moment J.D. told Dr. Cox of Ben's death, the show and J.D.'s narration revolves around the topic usually linked with 5 stages of grief: Denial, Anger, Bargaining, Depression and Acceptance.
- Seventh death in the series as well as the second death of a family member which is Jordan's brother Ben.
- Correction: eighth death in the series.
- The idea of Ben's untimely death is alluded to when Dr. Cox questions the lack of his camera in the final act of the show. At the beginning of the episode, Ben responds to Cox asking if he's still doing the whole camera thing with, "til the day I die." When JD delivers the news of his death, Ben appears from that point on without his camera.

My Butterfly

- The butterfly theme refers to "the butterfly effect" used in chaos theory, where a very small change in the initial settings may create a significantly different outcome. While the change in this episode is where the butterfly lands, "the very small change" is figurative and does not really have to be butterfly-related.
- JD and Dr. Cox's patient dies from an aortic dissection. This is the same condition that killed John Ritter, who had previously appeared as JD's father.

- Turk's middle name is revealed by J.D. as Duncan. Turk denies J.D.'s claim that his father was a big fan of doughnuts.
- In the scene where the Janitor digs J.D. over his love with Elliot, the Janitor compared them with "Ross and Rachel". They are the names of the two of the main characters in Friends (1994), in which Bill Lawrence was credited for writing one episode for. In that show, Ross holds a doctorate in palaeontology and Rachel is a buyer/personal shopper. In this episode, they are known as "Dr. Ross and Rachel from bookkeeping".
- The song playing over the final scene is 'One Thing' by Finger Eleven.

My Best Friend's Wedding

- With George Takei at his side, Turk flips his phone open to answer it like it was a Star Trek communicator.
- This is the first time Dr. Kelso refers to Turk as Turkleton.
- The actress who plays Carla's eyebrowless sister is Joselin Reyes, the fraternal twin sister of actress Judy Reyes.
- Despite Turk having at least one sister and two brothers, one of which is meant to be his co-best man, none of his siblings seem to be in attendance at his own wedding. The only member of Turk's family seen in this episode is his mother. It is possible his other siblings could not attend the wedding due to other commitments.
- This is the last episode in which Dr. Grace Miller is featured. At the beginning of the next season, her character is not seen. She would later reprise the role for final episode of Season 8
